Always use the Capstone Return Material Authorization (RMA) forms. This will ensure that your returned material is properly received at Capstone Turbine Corporation. Fill out a copy of the Field Service Report (512298) completely. Describe the system and/or the component history and failure. Obtain a Return Material Authorization (RMA) for removed material from Capstone Turbine Corporation. Properly crate all materials for return to Capstone. Use Capstone issued crates when provided. Clearly, mark the RMA number on the outside of crate and/or package. DO NOT mark directly on the components. Attach a completed copy of the Return Material Authorization form and the Field Service Report (512298) with the returned package. USA and Canada: Return materials within 30 days from repair date. Arrange return shipments through NuBorn Express. You may contact NuBorn Express at: Europe: Return materials within 30 days from repair date.
