Always use the Capstone Return Material Authorization (RMA) forms. This will ensure that your returned material is properly received at Capstone Turbine Corporation. 􏰒 Fill out a copy of the Field Service Report (512298) completely. Describe the system and/or the component history and failure. 􏰒 Obtain a Return Material Authorization (RMA) for removed material from Capstone Turbine Corporation. 􏰒 Properly crate all materials for return to Capstone. Use Capstone issued crates when provided. 􏰒 Clearly, mark the RMA number on the outside of crate and/or package. DO NOT mark directly on the components. 􏰒 Attach a completed copy of the Return Material Authorization form and the Field Service Report (512298) with the returned package. 􏰒 USA and Canada: Return materials within 30 days from repair date. Arrange return shipments through NuBorn Express. You may contact NuBorn Express at: 􏰒 Europe: Return materials within 30 days from repair date.
